Sepsis Collaborative
Since April 2024, Qsource has collaboratively worked with five nursing homes across Indiana on improving hospital readmission rates related to sepsis.
Through monthly interactive learning sessions, participants were provided effective internal tracking mechanisms to monitor hospitalizations due to infection. Qsource reinforced the importance of resident and staff hand hygiene and provided observation tools. Educational resources were offered for residents, families, and staff on prevention and early detection of sepsis. Providers were encouraged to provide sepsis education during resident and family councils.
We integrated education in these learning sessions with a variety of guest speakers who presented on topics of what sepsis is; personal experience with sepsis; creating an effective partnership with the hospital; and how to incorporate palliative care and hospice services at the right time. In addition, each participating nursing home was provided onsite and virtual one-on-one assistance to discuss, monitor, and support their improvement activities. Qsource introduced Viven Health’s sepsis learning modules that all staff can access on their phone and/or computer.
Although the “project” is wrapping up this month, the providers plan to continue their efforts with Qsource by their side addressing sepsis-related hospital readmissions.
