Life Care Center of Athens joined Qsource's Culture of Safety Center (CSC) initiative in early 2023. The facility chose to focus on falls and falls with major injury. Life Care Center of Athens was averaging 30+ falls per month. Working closely with Qsource, the Administrator and Director of Nursing conducted a performance improvement project that resulted in an overall 10% reduction in the number of falls and falls with major injury.
By avoiding high-level citations and civil monetary penalties, Qsource saved the facility both time and money. The reduction in their falls percentage led to an overall increase in points for their long-term quality measurement scores, as well.
