Qsource ESRD Networks provide support and services to dialysis providers and patients across five states. Network 10 serves the ESRD community of Illinois and Network 12 serves Iowa, Kansas, Missouri, and Nebraska.
As part of our contract with the Centers for Medicare and Medicaid Services (CMS), we are committed to serve as change agents and partners in quality improvement, support EQRS management, provide technical assistance through both onsite and virtual means, and assist patients and facilities with patient grievances, maintain access to care, and review clinical quality of care concerns.
Patient Services Team
Our Patient Services team is here to improve the quality of life of ESRD patients through facilitation of grievance resolution and investigation of clinical quality of care concerns when needed. Our team also provides support to facilities who have concerns about access to care or behavioral challenges that some patients face.

Data Management Support Team
Our data team provides technical assistance to dialysis providers and transplant programs in each of our five states, including clinical data in the ESRD Quality Reporting System (EQRS) and forms compliance such as 2728, 2746, and the annual 2744 survey. More information can be found here.
Our QI team facilitates quality improvement strategies to assist facilities in meeting the metrics set forth by CMS to improve quality of life for dialysis and transplant patients.
