As you wrap up the end of the year, take some time to reflect back on all of the hard work you have done and what you have accomplished. Are you grateful for yourself? Do you spend time reflecting on how far you have come? View these quotes that inspire gratitude to spark this conversation with yourself.
During December, many people will be tying up loose ends, checking things off their to-do list, pulling decorations out of the attic, and spending time with loved ones. With all of the stress that the holidays can bring, our immune systems may need some extra attention.
Remember to keep yourself safe from germs as you travel to celebrate with family and friends by simply washing your hands regularly and taking time to slow down and check in with yourself to see how you are feeling. See the Flu Zone Tool for warning signs to look out for.
Transportation Best Practices
This is the last month we will include this Transportation Best Practices resource that includes facility responses on how to combat the transportation issues they are facing. Download the transportation resource and see if you can implement any of these ideas to ensure that each person can attend all appointments and dialysis treatments!
Have another way that your facility has been successful in helping patients get reliable transportation? Share your success story by emailing the ESRD Network.
This month we would like to recognize Lea Bailey who is the Clinic Manager for FMC West Plains Home Therapy, FMC Springfield North Home Therapy, and FMC Bolivar Home Therapy! Lea has been able to have regular participation in all three facilities' monthly QAPI meetings! This is a difficult Patient and Family Engagement goal to achieve. Thank you so much for recognizing the importance of it and working to accomplish this goal.
Do you have a facility staff member you’d like to nominate for the facility shoutout? Email us the details of who, what, and where at: Qsource-Peers@qsource.org
